Review all advance care planning tools and directives, including POLSTS or MOLSTS (depending on your state), Durable Power of Attorney, Wills, Trusts, Living Wills, Ethical Wills, etc.
Identify and name a healthcare proxy to make medical decisions in the event that you are not able to do so.
If you are currently facing terminal illness, frailty, or reduced independence, we'll explore care options to consider.
Additionally, I will provide hands-on assistance in completing the above tasks and offer resource referrals, as needed. You will come away from this feeling confident that you have taken important steps towards remaining in control of your future.
